If you’re diagnosed with inflammatory bowel disease (IBD), you might wonder how much of your condition was decided before you were even born, and how much was shaped by your life history. We know that our current body weight and our genetics play a role, but a massive new study has asked a deeper question: does the story of your physical growth, from the day you were born, through primary school, and into adulthood, leave a lasting mark on your gut health?
The study suggests that your physical growth history over time can modestly influence your risk of developing adult-onset ulcerative colitis. By mapping these life-long patterns alongside your inherited DNA, researchers are uncovering a clearer picture of how our bodies develop.
A Massive Scale and a Fifteen-Year Timeline
To trace these patterns, researchers analysed data from the UK Biobank, a large-scale biomedical database containing anonymous health records. The study was exceptionally large: it initially included 486,449 participants who were completely free of IBD at the start of the project.
To look specifically at growth over time, the scientists narrowed their analysis to 262,601 participants who had complete, reliable records of three key life stages:
- Their birth weight.
- Their recalled body size at age 10 (recalled during their adult assessments).
- Their measured adult Body Mass Index (BMI), which is a standard calculation of body fat based on height and weight.
The researchers followed this massive group of people for a median of 14.7 years. During this fifteen-year period, 1,722 participants developed Crohn’s disease and 3,340 developed ulcerative colitis.
Mapping the Growth Timeline (The Technology Explained)
To make sense of decades of growth data, the research team used an advanced mathematical method called group-based trajectory modelling.
This is a smart statistical data-analysis technology that acts like a growth tracker. Instead of looking at a single weight measurement, a computer algorithm analyses how a person’s body size moves up and down over their life and groups people who shared a similar journey.
The algorithm identified five distinct life-long growth paths (trajectories). When they compared these paths, they found two specific growth journeys that were linked to a slightly higher risk of developing ulcerative colitis:
- The Up-and-Down Journey: Starting with a normal birth weight, becoming thinner than average at age 10, and then becoming overweight or obese as an adult.
- The Heavy-Start Journey: Starting with macrosomia, which is the medical term for a baby born weighing much more than average (typically over 4 kilograms or 8 pounds 13 ounces), transitioning to an average size at age 10, and then becoming overweight or obese in adulthood.
Interestingly, these physical growth paths were modestly linked to adult-onset ulcerative colitis, but the researchers did not find any similar link for Crohn’s disease.
Genes vs Growth: The Combined Risk
But what about the genetic code you inherited? To find out if genes could override your growth history, the researchers calculated a polygenic risk score (PRS) for each participant.
A polygenic risk score is an advanced genetic scorecard. Rather than looking for a single “disease gene,” a computer algorithm analyses thousands of tiny variations across your entire DNA and adds them up to give you a single score showing your inherited vulnerability to IBD. It acts as a personalised genetic risk rating.
When the scientists stratified (grouped) the participants by their genetic risk score, they discovered something fascinating. The risk of developing ulcerative colitis was slightly elevated for certain growth paths even in people who had a low genetic risk scorecard. This means your growth trajectory over your life may contribute to your risk independently of your inherited genetic susceptibility.
