When you are diagnosed with Crohn’s disease or ulcerative colitis, getting control over your gut inflammation as quickly as possible is the single most important step for protecting your long-term health. For many years, traditional care followed a “step-up” approach, starting with older treatments or repeated steroid courses and only moving to advanced biologics after symptoms returned.
However, research presented at Digestive Disease Week (DDW) 2026 and highlighted in Gastroenterology & Endoscopy News by Dr Miguel Regueiro, Chief of the Digestive Disease Institute at Cleveland Clinic, proves that care is shifting. By using powerful advanced therapies early (a “top-down” approach) and developing a brand-new class of drugs called TL1A inhibitors, medical science is opening up unprecedented levels of deep gut healing.
Breakthrough 1: Tulisokibart and the Promise of ‘Disease Clearance’
The first major trial highlighted in the report is the ARTEMIS-UC study, a phase 2 and phase 3 clinical trial evaluating a novel medication named tulisokibart.
Tulisokibart belongs to a brand-new class of biological medicines called TL1A inhibitors (Tumour Necrosis Factor-like ligand 1A inhibitors). TL1A is a key protein signal in your body that drives both active gut inflammation and tissue scarring. By blocking this specific signal, tulisokibart helps calm overactive immune cells while protecting the bowel wall from permanent damage.
In the initial 12-week double-blind trial, adult patients with moderate to severe ulcerative colitis received intravenous infusions of tulisokibart (1,000 mg on day 1, followed by 500 mg at weeks 2, 6, and 10) or a placebo.
By week 12, patients receiving tulisokibart achieved impressive results compared to the placebo group:
- Stool Frequency Normalisation: 57.4 per cent vs 17.9 per cent.
- Rectal Bleeding Resolution: 61.8 per cent vs 34.3 per cent.
- Symptomatic Remission: 47.1 per cent vs 13.4 per cent (with noticeable improvements starting as early as week 2).
- Disease Clearance: 25 per cent vs 1.5 per cent.
To understand why this matters, it helps to break down what doctors mean by disease clearance. Traditionally, clinical trials only measured whether a patient felt better. Disease clearance sets a much higher “gold standard” by combining three milestones at once: zero daily symptoms (symptomatic remission), a healthy appearance of the bowel lining during a camera check (endoscopic remission), and zero microscopic inflammation in tissue samples (histological improvement).
In the long-term open-label extension period following patients through 50 weeks, disease clearance rates continued to climb, reaching 31.8 per cent in patients taking a 100 mg maintenance dose and 44 per cent in those taking a 250 mg dose.
Breakthrough 2: Top-Down vs Step-Up Therapy (The PROFILE Study)
The second late-breaker reported by Dr Regueiro is the 4.5-year readout of the landmark PROFILE study.
While many clinical trials only follow patients for a few months, the PROFILE study tracked 386 newly diagnosed Crohn’s disease patients across multiple hospitals for an average of 1,352 days (approximately 4.5 years). The trial directly compared two care strategies:
- Step-Up Therapy: Starting with conventional treatments or steroids, only stepping up to advanced biologics if the disease worsened.
- Top-Down Therapy: Starting advanced biologic treatment immediately after diagnosis.
The long-term results were striking:
- 80 per Cent Reduction in Surgery: Patients in the step-up group were 5 times more likely to require abdominal surgery due to Crohn’s complications compared to those who received top-down treatment (Odds Ratio 5.00).
- Fewer Hospital Admissions: Patients on step-up care experienced nearly three times as many hospital admissions (44 admissions) compared to those on top-down care (15 admissions).
- Halving Disease Progression: Overall, starting advanced therapy early halved the risk of disease progression over 4.5 years (Odds Ratio 2.61).
Dr Regueiro emphasised that while repeated courses of corticosteroids can temporarily mask symptoms, they carry severe long-term side effects and do not lead to lasting disease clearance. Treating IBD early with advanced therapies offers far greater safety and long-term protection.
Why This Matters to You as a Patient
If you are managing Crohn’s disease or ulcerative colitis, these combined breakthroughs offer an empowering, reassuring message for your care:
- Proof That Early Advanced Treatment Protects Your Future: This study gives you the scientific proof that starting an advanced therapy early after diagnosis gives you the best chance of avoiding future hospital stays and major surgeries.
- A New Era of Complete Healing: The success of TL1A inhibitors like tulisokibart shows that future treatments will aim for total “disease clearance,” helping your gut lining heal completely at a microscopic level.
- Empowerment to Question Repeated Steroids: If you find yourself taking repeated courses of steroid tablets to manage ongoing flares, you have every right to discuss switching to an early personalised advanced therapy plan with your gastroenterologist.
By choosing proactive, early care and embracing new targeted drug classes, you can protect your gut and enjoy a healthier, flare-free future.
