Combining Ustekinumab with Liquid Nutrition Boosts Gut Healing in Crohn’s Disease

When you are diagnosed with moderate to severe Crohn’s disease, finding a treatment strategy that calms active flare-ups and heals your gut lining is the most important step toward getting your life back on track. Advanced biological medicines have transformed how doctors treat gut inflammation, but medical scientists are constantly searching for ways to make these therapies work even better.

Now, a new study published in Nature, Scientific Reports and reported by European Medical Journal reveals an effective way to boost biologic treatment. By pairing an advanced biologic drug named ustekinumab with a temporary 16-week period of special liquid nutrition, researchers achieved more than double the rate of complete gut-lining healing compared to using the drug alone.

Demystifying the Terminology: Ustekinumab and Exclusive Enteral Nutrition

To understand why this combination works so well, it helps to break down the medical treatments and technology used in the trial:

  • Ustekinumab (STELARA): A targeted biological medication made from living cells. It acts like a precision key that blocks two specific inflammatory signals, interleukin-12 (IL-12) and interleukin-23 (IL-23), stopping overactive immune cells from attacking your bowel wall.
  • Exclusive Enteral Nutrition (EEN): A specialised, liquid-only medical diet. For a set period, a patient drinks a balanced, nutrient-rich formula as their sole source of nutrition while avoiding solid food. EEN provides essential vitamins and calories while resting the gut, calming inflammation, and repairing friendly gut bacteria.
  • Mucosal Healing: The gold-standard goal of modern IBD care. It means that during a camera check (an endoscopy), a doctor sees that the ulcers and swelling on the inner lining of your bowel have completely healed, leaving behind smooth, healthy tissue.

Inside the Four-Year Study: Scale and Setup

This research was a moderate-scale multicentre retrospective cohort study. Researchers evaluated data from 114 adult patients with moderate to severe Crohn’s disease treated across three specialised inflammatory bowel disease centres between October 2020 and September 2024.

All participants received standard, optimised ustekinumab therapy, starting with two initial intravenous drip infusions. The patients were divided into two well-matched groups to compare strategies over a 52-week (one-year) monitoring period:

  • Combination Group: 56 patients received ustekinumab alongside 16 weeks of concurrent Exclusive Enteral Nutrition (EEN).
  • Monotherapy Group: 58 patients received ustekinumab on its own without liquid nutrition therapy.

The Findings: Doubling Mucosal Healing Rates

By tracking patient recovery at 16 weeks and again at one year, researchers discovered that adding 16 weeks of liquid nutrition provided a dramatic boost to treatment success:

  1. Higher Initial Remission at Week 16: By week 16, 89.3 per cent of combination patients reached clinical remission (feeling completely symptom-free), compared to 55.2 per cent of patients taking ustekinumab alone.
  2. More Than Doubling Mucosal Healing: Complete mucosal healing on the bowel lining was achieved by 90.5 per cent of combination patients at week 16, compared to just 37.3 per cent in the monotherapy group.
  3. Sustained Relief at One Year (Week 52): The benefits of the 16-week liquid diet remained strong long after patients returned to normal eating. At week 52, 80.4 per cent of combination patients stayed in clinical remission, and over 71 per cent maintained complete mucosal healing across both the colon and the ileum.
  4. Safety: No severe side effects were reported, proving that combining liquid nutrition with biological therapy is a safe, well-tolerated approach.

Why This Matters to You as a Patient

If you are living with Crohn’s disease and preparing to start or adjust a biologic treatment, this study offers vital encouragement for your care plan:

  1. Proof That Nutrition and Medicine Work Together: This study proves that combining a temporary medical diet with your biologic drug can dramatically increase your chances of reaching deep, lasting gut healing.
  2. Protecting Your Gut Lining for the Long Term: Achieving complete mucosal healing early in your treatment protects your bowel from long-term scar tissue, reducing your risk of future hospital stays or surgery.
  3. A Practical Tool You Can Discuss: While drinking a special liquid formula for 16 weeks takes dedication, knowing that it more than doubles your chances of mucosal healing gives you a powerful, proactive option to discuss with your gastroenterologist and dietitian.

By partnering advanced biological medicines with targeted medical nutrition, science is opening new pathways to give you the best possible chance of long-term remission.
